Workplace accidents modifications to machines on-site

A tragic example. A workplace death in Berkshire
In 2011 Reading Crown Court heard that a man died in a Berkshire workplace accident when he became trapped in automatic inspection machinery used to measure the thickness of rolls of rubber and textile.

The machine had been modified 12 months earlier and the employer had failed to account for the the risk of workers accessing a dangerous part of the machine – the part in which the deceased worker became trapped. He was pronounced dead at the scene.
A HSE investigation discovered that the factory did not have a full-time health and safety manager on-site. Furthermore, no risk assessment had been performed post-machine modification. It was also found that the machine should have guards in place.

Additionally, the investigation revealed failings on five other machines on-site.

An HSE inspector commented, "This needless tragedy is typical of what can happen when health and safety management systems fail. Had the hazards been assessed, the lack of any guarding would have been highlighted, and this death could have been avoided.

"Companies should realise that when there are flagrant breaches of health and safety law HSE will prosecute."
