The spinal motion during whiplash

When a car accident occurs the neck is thrown around in such a way that a whiplash injury occurs. Studies have revealed that a specific pattern of neck movement occurs during car accidents. During a rear-impact car crash the sixth cervical vertebra (C6) moves back into extension before the upper neck starts to move. When C6 comes to the end of its extension range C5 starts to extend also. This creates an S shape, with the lower neck in extension and the upper neck in some flexion. This may lead to the upper neck extending, the whole neck then ending up in a C-shape, facing backwards. This whole process is sudden and occurs in less than 200 milliseconds.

This theory gives insight into why the C5/C6 parts of the spine are those most affected when a neck injury such as whiplash occurs. Other common injuries include disc disruption, stretched joint capsules and damage to the facet joints.

The S-shaped position caused by the whiplash motion can result in the upper neck joints rubbing together which could lead to headaches and neck pain. The lower cervical spinal levels are thought to go further than the normal maximum extension movements in Whiplash Associated Disorders, leading to the rear joints being forced together and the front of the vertebrae being overstretched.

Upper neck injuries, specifically whiplash injuries, although possible in low speed collisions are found to be more common in higher speed impacts.
