Whiplash treatments trial revealed cheap collars as good as active therapy

After suffering a whiplash injury there may be several ways of treating the pain that is experienced as a result of soft tissue damage in the cervical spine and adjacent neck muscles. Recommended treatments range from rest and painkillers to expensive bouts of holistic therapy involving traction tables and physiotherapy.

However, in 1990 results were published from a study into the early management of whiplash injury which concluded that active treatment such as physiotherapy has no significant benefit when compared to standard treatment.

The 20-week trial involved 135 whiplash patients who were randomly assigned therapies. Of those patients 74 received standard treatment and 61 had active therapy.

Standard treatment consisted of two weeks rest in either a soft or moulded collar, followed by a programme of unsupervised mobilisation in the form of gentle exercises.

Active treatment involved supervised sessions of traction using weights combined with advice on neck care and sleeping posture. Active treatment candidates were also advised to perform simple neck and shoulder exercises between therapy sessions.

The trial findings suggested that when comparing the standard method of whiplash treatment with the more expensive active treatment no benefits were found in the active therapy group at either short term (six to eight weeks) or medium term (five months) patient reviews. Patients in the standard treatment group using moulded collars experienced greater pain relief and therefore, moulded collars in neutral or slight flexion are recommended by the trial report authors.
