Familiar scenario of surgical error
A 26-year-old woman has to have her first baby delivered by emergency caesarean section in December 2007. She is sent home after a successful birth. However, several weeks after the operation she is suffering stabbing pains to the right side of her stomach.
On re-admittance to the hospital which performed her original operation, she is forced to undergo another emergency procedure on Christmas day to remove the surgical swab which had been left inside her after the birth of her baby.
She told a national newspaper, "I'm really angry that they took away my first Christmas with my first baby and partner… and with such a simple error."
Wrong site surgery
A 62-year-old cancer patient undergoes a surgical procedure to drain fluid from her right lung in 1999. But her Doctor inserts the needle into the left lung. On realising his mistake he quickly swaps surgery sites, but the woman dies a few hours after surgery from a heart attack.
The General Medical Council later found the doctor guilty of serious professional misconduct after he tried to cover up his medical negligence.
Guidance to prevent surgical error
In 2005 new guidance was issued by The Royal College of Surgeons and the National Patient Safety Agency calling for the clear marking of surgical sites on patients' bodies to be made in a standard style, with indelible ink, while the patient is conscious, and for a pre-operative checklist to be observed by the whole surgical team involved in the procedure.
It was believed that a non-standardised system of site markings had led to some surgical errors as surgeons moving from hospital to hospital around the country could be confused by different body markings.
