In August 2009, a case came to light where a Primary Care Trust (PCT) lost the notes of a patient with heart disease and after a three year legal battle the trust admitted liability for the negligent treatment that the man received as a result.
The 57-year-old from Stoke-on-Trent had been suffering from heart disease which required medication and after his PCT-managed doctor's surgery lost his medical notes his treatment was inexplicably changed.
His widow told local press that it was not until after her husband's death in 2006 that they were able to get any information.
She said, "We knew something wasn't quite right with his care, but couldn't find what it was until after he died."
In a letter to the widow a chief executive of the PCT apologised for the fact that her late husband did not receive appropriate secondary prevention in respect of his heart disease and acknowledged that the right treatment would have prolonged his life.
Lawyers for the PCT said in a statement that it was understood that "the deceased would have survived an additional six months".
At the time of writing, the legal process was still ongoing and no-one from the Stoke-on-Trent PCT would comment on the medical negligence claim.
