Prevention of a ladder accident should be a simple matter of the user, his or her employer and anyone else concerned in the task putting easy measures in place to reduce risks of a fall.
These precautions are likely to include securing the ladder to a stable surface at the top and, if a particularly long ladder, mid-way along its length. Its feet should also be prevented from slipping, either by technical means such as stays and grippers or human action halting possible movement.
Modern suction pads and grippers of various styles are generally efficient at holding a ladder's feet steady but there can be circumstances where they will not be effective enough to prevent a workplace accident and potential for personal injury being suffered by the employee or contractor who is using the ladder.
An instance of this could be where the floor on which the ladder is resting is particularly slippery. It may be that hygiene considerations or other issues relating to the area's activities mean the surface is unsuitable and unsafe for the bottom of the ladder to be firm and secure.
If this is the case, a work-related accident may occur if the ladder-user does not realise it could be a problem and climbs the rungs, thinking the operation is without risk but the feet of the ladder then slip, causing him or her to fall from height.
Determining liability for such an accident will depend on a number of factors: whether any warnings about the slippery floor were given to the ladder-climber; who provided the ladder and if any safety instructions were issued to the user; who trained and supervised the ladder-user; whether the ladder provided was suitable for the task and if any other action could have been taken to prevent the injury occurring.
One action which could prevent the accident or reduce its seriousness would be to have a second work colleague holding the ladder feet or in some other way holding the equipment steady to halt its movement on the floor.
If management or another supervisor of the work has suggested a second worker is unnecessary or authorised short-cuts in completing the work which might compromise the safety of a ladder-user then that person or organisation could be deemed partly responsible for an accident.
