Asbestosis and the HSE register of asbestos-related deaths

In order to maintain statistics on deaths caused by asbestos exposure, the Health and Safety Executive (HSE) keeps a register of all deaths where asbestosis is mentioned on the death certificate.

The register includes deaths where the underlying cause has been determined as asbestosis and also those where asbestosis is mentioned in any written description of other causes and conditions which may have contributed to death.

In terms of a death certificate "the underlying cause" is defined as the disease or injury which initiated the train of morbid events leading directly to death.

So, the asbestosis register is useful in ascertaining how many individuals were suffering from the industrial disease even if the direct cause of death was another disease.

Asbestosis a definition
Asbestosis is a lung disease caused by exposure to asbestos fibres which, once inhaled, cause fibrosis of the parenchymal tissue of the lungs. The inflammation in the lungs causes sufferers to experience symptoms including shortness of breath, a cough and chest pain.

Occasionally asbestosis sufferers find that the ends of their fingers become swollen, misshapen and red due to a build of fluid in the tissue of the fingers. This is called finger clubbing.

Asbestosis is specific in that its diagnosis refers to interstitial fibrosis in the main structure of the lungs and not fibrosis of the pleura (the membrane surrounding the lungs), or pleural plaquing (scarring of the lung lining).

A diagnosis will be made based upon clinical factors, X-ray results and if a history of exposure to asbestos is presented. Generally, it is recognised that in order to develop asbestosis the sufferer must have been heavily exposed to asbestos, although this may have been up to several decades beforehand.

The interpretation of figures for cases of asbestosis is complicated by two main issues:

1 Death may sometimes not be recorded as having resulted from asbestosis as it may have gone undiagnosed or have been mistaken for other types of lung fibrosis and disease.

2 The term asbestosis, may have been mentioned on a death certificate inaccurately to indicate the substance of asbestos as having a role in another type of lung disease, such as mesothelioma for instance. On some death certificates the phrase "industrial disease of asbestosis" is used when the person has had mesothelioma.
