The same is true of inspections - if a worker suffers personal injury as the result of defective equipment that the employer should have recognised as being substandard, his or her employer may find themselves liable because of their negligent inspection procedures.
Effect - or "actually get on with it" - is the final stage, hopefully with the appropriate levels of safety in place as a result of the preceding five steps.
