Brewery forklift truck accident
In August of 2011 the Health and Safety Executive issued a warning to employers of the ever-present danger of fork lifts.
The warning followed an incident in which a 64-year-old delivery driver suffered fatal injuries after being hit by a forklift truck while making a delivery.
HSE Inspector Lyn Spooner commented, "This incident occurred because of inadequate risk assessments, poor management and monitoring of contractors, and managers failing to understand their responsibilities for health and safety."
Shipyard forklift truck accident
In the same year a 62-year-old man suffered fatal crush injuries after becoming trapped between the forklift he was driving and a steel girder.
An inquest heard that the man was a welder and was not trained to drive the forklift. The company's training records were described as "ad hoc" and it was also heard that the forklifts did not have stickers warning workers that only authorised and trained staff should be permitted to drive them.
