Keeping a personal injury diary
If you have been involved in an accident that has resulted in you being injured, you may be able to make a personal injury claim, if the accident was caused by someone else’s negligence. (See Can I claim for more information).
If you intend to make a claim for compensation, it will greatly help your case if you keep a record of the effect that the accident has had on you, as you recover.
This can be done by keeping a personal injury diary ( click here to download a free copy).
The document is in PDF format so 'Adobe reader' will be required to view it:
If you instruct YouClaim to help you with your accident claim, we will ask you to start an injury diary, to keep a record of your recovery. We will ask you to record a few lines each day, covering:
- How your symptoms and injuries develop and
your recovery over time
- Any other health related effects of the accident
(eg: insomnia, flash backs, depression)
- Medication taken and dosage
- Doctor’s appointments
- Treatment
- Out of pocket expenses (eg: prescription costs,
taxi fares, phone calls)
- Days off work unpaid
- The effect on your normal daily activities
(eg: unable to wash or dress unaided,
unable to play football or socialise etc.)
Part of our claim process involves obtaining an independent medical report on the effect the accident has had on you, how well you have recovered from it, and any ongoing or permanent symptoms you have. We will arrange for you to see a doctor, close to where you live.
They will ask you questions about the accident and your injuries and you should take your diary with you to show the doctor. The medical report will be used to support your claim for compensation, so you should continue with your diary until you have attended this appointment (or until your symptoms subside, if this is sooner).
See Claiming compensation, for more information about our claim process.
