Home birth - once a birth injury compensation fear, now a viable option

Until relatively recently hospitals have tended to resist the desires of women wishing to have a home birth for fear they would, in the event of something going wrong, risk liability for birth injury compensation.

However, a Dutch study, the most comprehensive yet examining the issue, has revealed that for low-risk mothers-to-be, the risk of complication is negligible, comparable even to that of women having hospital births.

While this study, the work of the Dutch Ministry of Health and reported in the British Journal of Obstetrics and Gynaecology, was exhaustive, examining nearly 530,000 births over a 7-year period, it should be noted that it only details the Dutch experience of a limited cohort: low-risk women being cared for by a midwife at the time of labour.

While it would be difficult and perhaps careless to infer that the study would correlate to be representative of the UK experience, as many factors in maternity care can be population or health system specific, it certainly raises some questions that are likely to be of interest to the NHS.

For the purposes of the study the definition of "low-risk" excluded a great deal of women, including those who needed pain relief, those who required induction or foetal monitoring and those who had had a difficult previous pregnancy, perhaps of the kind that eventually necessitated the making of a claim for birth injury compensation.

Most significantly, the study found that there was only negligible difference between the rates of perinatal mortality when comparing statistics for home births with statistics for hospital births.
