Symphysiotomy is a surgical procedure performed during childbirth to widen the pelvis by cutting the cartilage of the symphysis pubis (the midline cartilaginous joint which unites the left and right pubic bones).
First advocated in 1597 by Severin Pineau symphysiotomies were at one time a routine procedure for women suffering obstructed labour. However, as the prevalence of Caesarean sections increased towards the end of the 19th century, the symphysiotomy became a much rarer procedure.
Symphysotomy provides a temporary increased pelvic diameter of up to 2 cm. The surgical division of the symphysis ligaments should be carried out under a local anaesthetic and then the baby must be delivered using vacuum extraction methods.
The birth intervention procedure is now rarely used in developed countries, but can be a lifesaving course of action where C-section is not feasible.
However, there are inherent risks associated with symphysiotomy, such as urethral and bladder injury, infection, long term difficulty in walking and pain.
Between the 1950s and 1980s many women in Ireland unknowingly underwent symphysiotomies during childbirth and were left with severe side effects, including extreme pain, incontinence and depression. It is said that Irish obstetricians were seeking to establish the procedure as an alternative to Caesarean section as it was feared that women made to undergo repeated C-sections might seek the use of contraception and therefore act against their Catholic beliefs.
